Our 5 year strategy

We want to make sure that local people have the best possible healthcare now and in the future. So we must continue to spend our money wisely if we are to also achieve good improvements in health and wellbeing for everyone in Southport and Formby. 

Our 5 year strategy sets out how we will do this. it has been developed jointly with our neighbours in NHS South Sefton CCG and it describes where we will focus our efforts towards improving the lives of our local residents.  

The work needed to improve health and wellbeing will mostly be specific to Southport and Formby but we recognise that we cannot achieve our vision on our own. 

This is why our strategy is Sefton wide, so we can work more easily with our partners right across health and social care.

“We want all health and care services to work better together – to be more joined up – with as many as possible provided in our local communities, so it is easier for you to get the right support and treatment first time, to help you live a healthy life and improve your wellbeing.” 

We call this vision community centred health and care and it is at the heart of our overarching Shaping Sefton transformation programme.

Why things need to change

Demands on health and social care are increasing and locally there are a number of reasons why this is the case. Here are just two:

  • We have a growing number of older residents with more complex health conditions, and this is much higher than the national average
  • We know that residents living in some parts of the borough can expect to live unacceptably shorter lives than their neighbours in more affluent areas of Sefton

Who we are working with

The organisations we worked with to develop our strategy include Sefton Council's Health and Wellbeing Board, Healthwatch Sefton, NHS organisations like hospitals and other CCGs, Sefton CVS and local voluntary, community and faith groups. 

Patients, carers and public have also helped us to understand what we need to do to improve health and health services, and their views have also informed our 5 year strategy.

What next?

Our Shaping Sefton programme will help us to deliver our vision for future health and care. It is underpinned by a number of blueprints that focus on those priority areas of health and care where we believe we can make the biggest difference.
