What is Shaping Sefton II?

‘Shaping Sefton II’ is our five year plan for working in a more joined up way across local health and care partners so that patients can get the right support and treatment first time, to help them live a healthy life and improve their wellbeing.

We are doing this as part of Sefton Health and Care Partnership (SHCP), which is in the process of building on the successes of our original Shaping Sefton strategy to address some of the biggest health and care challenges we are facing together between now and 2024.

Shaping Sefton II will support the delivery of the priorities set out in our local Health and Wellbeing Strategy, currently being refreshed by Sefton Council. The plan will also support the work of the Cheshire and Merseyside Health and Care Partnership in developing its response to the national NHS Long Term Plan.

How are we engaging?

We have based our thinking our thinking so far on all the information we have about people’s health and care needs in the borough, including the priorities identified in the Sefton Joint Strategic Needs Assessment (JSNA), which outlines the health and care needs of everyone in the borough.

SHCP is encouraging as many people as possible to have a look at the new draft plan, tell us what you think and more importantly let us know if we have got our priorities and ambitions right. We also want to know if we have missed anything You feel is important about future health and care.

The team from SHCP has been out and about speaking with our residents at various events and meetings, including our annual Big Chat 11 AGM event in September and we are also asking people to fill in an online survey to gather views.

What happens next?

We are asking for your feedback by Monday 21 October and all of the feedback received will be used to finalise Shaping Sefton II, which we plan to publish later this year.
